yes! i am able to offer sessions for anyone living in arizona
do you take insurance?
i am not able to offer insurance through shape change counseling. however, through my supervisor ej millstone (move toward good) i can accept blue cross blue shield and aetna. if your interested in using insurance, please email me at shamilton@movetowardgood.com
what is your out of pocket rate?
my out of pocket rate is $150 per session with sliding scale options available

i am a Licensed Associate Counselor, LAC- 23396, and this practice is currently supervised by the wonderful ej millstone.
do you write gender affirming surgery letters?
oh hell yes I do, and would be happy to write one for you. in order for this to happen we will need to schedule a 15-minute free consultation. After meeting, please allow a week to receive your letter.
